Description

RN: Working in conjunction with the Director of Nursing and Nurse Manager, the RN assesses, plans, implements and collaborates with other departments to ensure excellence in resident and family care, reflective of CCMH mission, vision and values. The RN will integrate departmental goals for improvement across the care continuum. She/he coordinates and delegates Neighborhood and resident care activities, as appropriate, to other health care providers. The RN practices within the framework of the Wyoming Nurse Practice Act, American Nurses Association professional standards, CMS, National Resident/Patient Safety Goals, and ANC Standards of Care

PRIMARY JOB DUTIES

  • Performs resident assessment/evaluation and documents in resident record.
  • Develops an age appropriate plan of care, which addresses physical, psychosocial and spiritual needs, in collaboration with other members of the health care team.
  • Implements the plan of care, delegates appropriately and ensures documentation of interventions.
  • Evaluates progress towards desired outcomes, revises plan of care and intervenes as necessary.
  • Educates resident and families regarding current health status, plan of care, and anticipated outcomes.

LPN: Working in conjunction with the Associate Nurse Manager, Director of Nursing and the RN, the LPN assists in all aspects of the nursing process according to CCH and department policies and procedures. The LPN collaborates with other departments to ensure excellence in resident and family care reflective of CCH mission, vision and values. She/he coordinates and delegates neighborhood and resident care activities, as appropriate, to other health care providers. The LPN practices within the framework of the Wyoming Nurse Practice Act, CMS, National Resident/Patient Safety Goals, and professional standards.

  • Participates in development of an age appropriate plan of care, which addresses physical, psychosocial and spiritual needs, in collaboration with other members of the health care team.
  • Implements the plan of care, delegates appropriately and ensures documentation of interventions.
  • Assists with patient/resident and family teaching regarding current health status, plan of care, and anticipated outcomes.
  • Assists with evaluating patient/resident response to intervention and revision of plan of care.


Qualifications

RN: Education: Graduate of an accredited school of nursing.

Licensure: Wyoming Registered Nurse licensure.

Must have current Healthcare Provider CPR or Red Cross Professional Rescuer certification within 60 days of employment.

Experience: Experience in long term /skilled care and/or medical/surgical nursing preferred.

LPN: Education: Graduate of an accredited school of practical nursing.

Licensure: Current Wyoming license as a practical nurse.

Must have current Healthcare Provider CPR or Red Cross Professional Rescuer certification within 60 days of employment

Experience: 1 year preferred in long term/skilled nursing care and/or medical/surgical nursing.
