Description

The Minimum Data Set (MDS) Coordinator works under the supervision of the Associate Nurse Manager, Rehabilitation Coordinator and the Director of Nursing to assure that the federally mandated MDS process is completed accurately and timely for the State of Wyoming. MDS coordinator plays a critical role in the development of the total plan of care for each resident and ensures compliance with state and federal regulations. She/he implements and collaborates with other departments to ensure excellence in resident/patient and family care, reflective of CCMH mission, vision and values. Ensures the integration of departmental goals for improvement across the care continuum. The MDS Coordinator practices within the framework of the nursing process in the provision of professional nursing care within the Wyoming Nurse Practice Act, CMS, and American Association professional standards.



Qualifications

Graduate of an accredited school of nursing, current Wyoming Registered Nurse licensure. RAC-CT (Resident assessment coordinator certified) recommended. Experience in medical/surgical/skilled/geriatric nursing preferred, CPR required, CPI/Behavioral Management training as indicated
