Description

RN - Working in conjunction with the Practice Manager and Clinic Providers, the RN assesses, plans, implements and collaborates with other departments/services to ensure excellence in patient and family care, reflective of Campbell County Health (CCH) & Campbell County Medical Group's (CCMG) mission, vision and values. The RN will integrate clinic goals for improvement across the care continuum. She/he coordinates and delegates patient care activities, as appropriate, to other members of the health care team. The RN practices within the framework of the Wyoming Nurse Practice Act, American Nurses Association professional standards and National Patient Safety Goals.

PRIMARY JOB DUTIES

  • Performs patient assessment and documents in patient record.
  • Utilizes the nursing process to adapt interventions to meet age appropriate care, including physical, psychosocial and spiritual needs (when appropriate), in collaboration with other members of the health care team.
  • Implements and evaluates care interventions, delegates appropriately and ensures documentation of interventions.
  • Evaluates progress towards the prescribed plan of care and coordinates the care with clinic providers and intervenes as necessary.
  • Educates patient and families regarding current health status, plan of care, and anticipated outcomes.
  • Performs other duties as assigned.

LPN - Working in conjunction with the Practice Manager and Clinic Providers, the LPN assists in all aspects of the nursing process according to CCH and departmental policies and procedures. The LPN collaborates with other departments to ensure excellence in patient care reflective of CCH mission, vision and values. The LPN will integrate clinic goals for improvement across the care continuum. She/he coordinates and delegates patient care activities, as appropriate, to other members of the health care team. The LPN practices within the framework of the Wyoming Nurse Practice Act, American Nurses Association professional standards and National Patient Safety Goals.

PRIMARY JOB DUTIES

  • Performs evaluation appropriate to licensure and documents in patient record
  • Participates in development of an age appropriate plan of care, which addresses physical, psychosocial and spiritual needs, in collaboration with other members of the health care team, including MA as directed by provider.
  • Implements the plan of care, delegates appropriately and ensures documentation of interventions.
  • Assists with patient and family teaching regarding current health status, plan of care, and anticipated outcomes.
  • Performs other duties as assigned.


Qualifications
RN - Education: Graduate of an accredited school of nursing.

Licensure: Wyoming license.

Must have current American Heart Association Healthcare Provider CPR or American Red Cross Professional Rescuer CPR within 60 days of employment.

Experience: Experience in ambulatory clinic nursing preferred.

LPN - Education: Graduate of an accredited school of nursing.

Licensure: Wyoming license.
