Home Health

Campbell County Health's Home Health provides care to patients at home—where they receive support from trained medical professionals, family, friends, plus the comfort of familiar surroundings.

Regardless of age, everyone appreciates the comfort and convenience of recovery in their home as an alternative to hospitalization or a nursing home. Home healthcare may also mean earlier discharge from the hospital, and lower healthcare costs. It is a personalized form of care, which may be paid by Medicare, Medicaid, private insurance, or directly by the patient.

Services

Our professional home care services utilize registered nurses, certified home health aides, physical, speech, and occupational therapists and medical social workers. Home Health is offered under the direction of the patient’s physician and our registered nurses are on call 24-hours a day, 7-days a week.

Home Health Telemonitoring

We’ve seen the commercials-a video “doorbell” that lets you see and hear who is at your door. That kind of instant communication is now helping patients at CCH Home Health with telemonitoring. The first service of its kind offered at Campbell County Health in Gillette, Wyoming, telemonitoring transmits clinical information like blood pressure from the patient’s home to trained home health nurses in about four minutes.

If the home health nurse determines that a patient could benefit from home monitoring, patients receive a kit that contains a scale, blood pressure cuff, pulse oximeter (to measure oxygen levels in the body) and a transmitter. Additional equipment, like a glucometer (a device that measures blood sugar levels) can be added to the kits. The patient is then trained to use the equipment until they feel comfortable doing it without a Home Health nurse.

Lifeline Personal Response System

With the simple press of a button, at any time of the day or night, a subscriber can be quickly connected with a trained professional. The professional makes voice contact, reviews the patient’s personal and medical profile, assesses the situation and then notifies the appropriate support and medical response team as required. Lifeline allows people of any age to move freely around their homes with the confidence of knowing they have help if they need it.
