The Community Health Needs Assessment (CHNA) describes the health status of the hospital’s community and is to be used by the health system and other collaborators when developing an implementation plan that addresses one or more identified community needs.
CHNA hard copies are also available at the Administration office of Campbell County Health.
From our CEO Matt Shahan, “Healthcare organizations across the country are increasingly being called upon to help support the priorities identified in their communities and states. The health issues in this document are closely aligned with what we know are concerns both locally and in Wyoming. We see this as an opportunity to work collaboratively with other agencies, groups and organizations to make tangible improvements in community health.”
