CCMH Billing Statement Feedback Wanted

The Patient Accounting department is asking for feedback from patients on a proposed new billing statement from July 1-August 1. The department has compiled comments made from patients in the past, as well as researched hospital invoicing trends to develop a new statement. They believe this proposed statement is a beginning of what a new statement should look like for patients, but are collecting comments for the month in order to make sure patients are satisfied with it.
A sample of the statement (compared to the old statement) and the survey can be found in the CCMH Cashiers area (where you can pay your bill), as well as online at ccmh.net/statement. The survey should take no more than five minutes to complete.
