Say hello to
Carlos C. De La Rosa, a certified nursing assistant (CNA) who works at CCH
Home Health and Hospice. Carlos has worked in health care as a CNA for more than 30 years—and
for the last five years he’s been with CCH also working in
Behavioral Health Services,
Intensive Care and
Medical/Surgical units. For those who don't know, CNAs are critical members of our
team. They fill a gap between patients and nurses. They help put patients
at ease, check and record vital signs, ask them about their conditions,
and create and maintain records, among many other tasks.
Some of you may recognize him from our
CNA campaign that we ran in 2016. Carlos became a CNA to "change his life for
the better and to help others," something we hear often of those
who choose a life-path in health care.
According to Home Health and Hospice Director Ashley Fraser, RN, CHPN,
“Carlos is an ‘out of the box’ thinker, always willing
to look at options to advocate for his patient’s needs. Carlos is
an exceptionally hard worker, who maintains an upbeat and positive approach
to patient care, even in the most challenging situations.”
Other colleagues have this to say about Carlos:
“Carlos is willing to help whenever and wherever is needed,”
says Hollie Lynde, RN, Home Health and Hospice. “He is dedicated
to his job and patients in all aspects of care. Carlos is good at finding
solutions to concerns that have not become problems yet—he is very
“Carlos truly goes above and beyond in his patient care as well as
being a resource to his co-workers,” says Jennifer Miller, RN, Home
Health and Hospice. “Carlos puts his patient’s at ease with
his skill and humor.”
Carlos calls Dr. Chuck Franklin his healthcare hero because, “He
is a genuine person in his professional life and his home life. He’s
a ‘what you see is what you get’ kind of a person. He is a
kind and caring person all the time.”
Outside of work, Carlos enjoys spending quality time with his family, as
well as fishing.
We asked Carlos what he likes most about working for CCH, and what excellence

“I’m proud to work for CCH and Home Health and Hospice because
we do a lot of good work here. I work with some amazing people. Here,
I know that my opinion matters, and that I am an important part of this team.
Excellence every day means that I’m doing my absolute best for everyone—my
patients, my colleagues and leadership. I’m so thankful for all
of the opportunities that CCH has given me.”
