List of Screenings
Wellness offers a daily screening program that makes it easy and affordable
for all residents to have their blood checked, or get other health screenings,
on a regular basis. When you use
My Health Home, you have access to your test results in less than 48 hours, unless otherwise noted.
Note: The Wellness Department can only perform the lab tests outlined in this
section; if the physician orders other tests for the patient, the patient
should visit the
Corporate clients and CCMH employees should talk to their health coach or Human Resources
representative for their company code.
Wellness Panel, $30: A total of 32 tests that measure cholesterol, glucose, thyroid, iron,
electrolytes, minerals, uric acid, triglycerides, liver function, and
CBC (complete blood count), $15: Different types of blood cells are counted and examined to present a
general picture of overall health and the person's ability to fight
infection and disease.
PSA (prostate specific antigen), $25: Measures a protein produced by the male prostate gland and recommended by the
American Cancer Society for men over age 50, or those with a family history of prostate cancer.
CRP, $35: In conjunction with cholesterol levels, CRP is a strong predictor of
peripheral artery disease. Highly sensitive CRP may also help identify
patients at risk of a first heart attack even with low to moderate lipid levels.
Hemoglobin A1C, $25: Also called Glycohemoglobin. Reflects average blood sugar levels over a
two-three month period.
Vitamin D, $35: Indicates possible deficiencies. Fat soluble vitamin that helps control
calcium and phosphate levels in the body, critical for growth of bones
Vitamin B12 & Folate, $30: Indicates possible deficiencies, anemias, and malnutrition or malabsorption.
HIV+, $30: Indicates presence of HIV infection.
iFOBT (Fecal Occult Blood Test), $20: A sample of stool tested for hidden blood indicating colon disorders including
colon cancer. A return envelope is provided to return to the
Blood Typing, $25: Determine your blood type: O+, O-, A+, A-, B+, B-, AB+, AB
Estradiol, $40: Estradiol levels are used in evaluating ovarian function.
FSH (Follicle Stimulating Hormone), $40: Often used in conjunction with other tests in the work-up of infertility
or when a women’s menstrual cycle has stopped or become irregular.
Progesterone, $40: Helps track ovulation, used in conjunction with other tests in the work-up
of infertility and supports gestation.
Testosterone, $40: In conjunction with other tests, testosterone is often used in the work-up
of a variety of conditions in men, women, boys and girls.
Celiac Screen (Tissue Transglutaminase IgA Ab)+, $50: Used to identify patients who have an increased probability of having
Celiac Disease. Should be used in conjunction with other tests to establish
diagnosis of Celiac Disease.
A gluten free diet prior to screening may have a negative result.
Hepatitis C Antibody+*, $50: Indicates the presence of a Hepatitis C virus infection.
Lead+*, $25: Used to determine the concentration of lead in the blood.
Thyroid Antibodies (Thyroid Peroxidase & Thyroglobulin)+, $40: Measurement of antibodies that assists in the diagnosis and management
of a variety of thyroid disorders.
T3, Reverse+, $ 55: Measurement of the inactive form of T3. Maybe useful for those with normal
Thyroid Panel results but experiencing symptoms of decreased thyroid production.
Thyroid Function Panel, $30: Thyroid stimulating hormone (TSH), Free T4 & Free T3 levels indicating
possible hyperthyroidism or hypothyroidism.
Diabetes Management Panel, $50: Includes the Wellness Panel and Hemoglobin A1C.
Iron Management Panel, $45: Includes iron, total iron binding capacity, unsaturated iron binding
capacity, and ferritin.
Men’s Health Panel, $65: Includes Wellness Panel, CBC, CRP and PSA. Recommended for men under
60 years old.
Savings of $30 vs. individual tests.
Women’s Health Panel , $65: Includes Wellness Panel, CBC, CRP and Free T4 for thyroid hormones and
metabolism. Recommended for women under 60 years old.
Savings of $20 vs. individual tests.
Senior Men’s Panel (age 60+), $85: Includes Wellness Panel, CBC, CRP, PSA, and take home iFOBT.
Senior Women’s Panel (age 60+), $85: Includes Wellness Panel, CBC, CRP, Free T4 for thyroid hormones and metabolism
and take home iFOBT.
Wellness Screen, $65: Includes Wellness Panel, CBC, Biometrics, and an easy to understand detailed
report. Biometric screenings are evaluations that look at your past, current,
and potential future health conditions. Included in this are blood pressure,
pulse ox (oxygen saturation levels), height, weight, and body composition.
The participant will have a clearer understanding of how they compare
to national statistics.

What to do before the screening
• Fast for at least 10 hours prior to your screening
• Drink plenty of water 24 hours prior to your screening
• Take medications as prescribed
• No strenuous exercise 10 hours prior to your screening
• Do not attend if you have severe cold or flu symptoms
• If possible, wear clothes with loose fitting sleeves
• If you are diabetic, schedule an early appointment
A Few Important Tips
• Blood work results are mailed directly to you.
• All tests conducted are for screening purposes only and do not
replace routine medical care.
• Payment by cash, check or credit card is required at the time of service.
• CCH Wellness will not file with your insurance company. Health
insurance may or may not reimburse for these screenings.
• It is your responsibility to send results to your healthcare provider.
