CCH Employee & Spouse Health Screenings

The Wellness Department is now the Occupational Health Clinic, located inside the Walk in Clinic at 501 S. Burma Avenue. This program is available to ALL CCH employees – full-time, part-time or PRN, with or without CCH health insurance. The program is also open to spouses of employees who are enrolled in the CCH health insurance. Learn more about the program

Screening Dates

  • September 1-30
  • Monday - Friday 6 am-12 pm
  • Occupational Health, located in the Walk-in Clinic at 501. S. Burma Ave.

What's Included

  • Health Screen: Wellness Panel, CBC (Please fast for 10 hours), PSA (for men > 50). Additional screenings available (self pay at time of screening). A list of tests can be found here
  • Optional: Audiogram screening (for CCH employees only); and Health Coaching for all CCH employees, and spouses on CCH insurance.

If the blood draw is not completed in September, insurance discount will not apply. CCH Wellness program participants who also participate in another company’s wellness program must screen individually with each company.

Registration

Registration opens August 8 at 9 am and closes at noon the day before each screening.

Online registration is preferred. Company Code is CCH2022.

Those who participate in multiple corporate wellness programs must screen with each company to be compliant with each companies wellness programs.

  • Step 2: Click ‘Make an Appointment’

  • Step 3: First time users click ‘Create an Account’

  • Step 4: Use Company Code CCH2022

  • Step 5: Follow the instructions on the screen to select a date and time.

    What to do before the screening

    • Fast for at least 10 hours prior to your screening
    • Drink plenty of water 24 hours prior to your screening
    • Take medications as prescribed
    • No strenuous exercise 10 hours prior to your screening
    • Do not attend if you have severe cold or flu symptoms
    • If possible, wear clothes with loose fitting sleeves
    • If you are diabetic, schedule an early appointment
