What is Healthy U?
Healthy U is the name for the Wyoming Center on Aging’s implementation
of Stanford University’s evidence-based self- management programs,
which include Chronic Disease, Chronic Pain, and Diabetes
Self-Management programs
- Healthy U workshops are led by trained peer facilitators. In Healthy U,
participants learn a variety of skills including: Techniques to deal with
frustration, fatigue, pain, and isolation
- Appropriate exercise for improving and maintaining strength, flexibility,
and endurance
- Appropriate use of medications
- Communicating effectively with family, friends, caregivers, and healthcare providers
- Nutrition
- Decision-making
- Action planning
- How to evaluate new treatments
Reliable health outcomes
Developed in 1996, CDSMP and its affiliated programs have been extensively studied.
Studies have yielded the following outcomes among program participants:
- Improved exercise habits
- Improved cognitive symptom management
- Improved communication with physicians
- Improved general health
- Decreased health distress
- Decreased fatigue
- •Decreased disability
- Decreased social and role activity limitations
- Fewer days in the hospital
- Fewer outpatient visits and hospitalizations per year
- Healthcare savings of approximately $364 per participant per year
Healthy U is available to patients all across Wyoming
Healthy U is coordinated statewide by the Wyoming Center on Aging in partnership
with the Wyoming Department of Health Public Health and Aging Divisions.
The program was brought to Wyoming in 2015. Healthy U is free to attend.
Any adult that wants to learn more about managing their health or who helps
manage a loved one's health is invited to attend regardless of whether
or not they have a chronic health condition themselves.
Diabetes
A free six-week health workshop for people managing type 2 Diabetes and
their friends, family and caregivers. Offered every Wednesday from January
19-March 2, 2022.
Learn to better manage your diabetes using evidence-based strategies. Topics include:
- Problem solving
- Decision making
- Managing your symptoms
- Healthy eating
- Action planning
- Exercise
- Effective communication
- Evaluation treatment
