Starting July 19, 2025, all of your personal health information from Campbell County Health (CCH) and Campbell County Medical Group (CCMG) will be available exclusively through Epic MyChart.
With MyChart, you get secure, 24/7access to your health records from any device with internet access. View lab and radiology results, visit summaries, upcoming appointments, immunizations, medications and more all in one easy-to-use platform.

Your personal health information (PHI) for services provided on or after July 19, 2025, will only be available through MyChart. Epic MyChart is just one more way we're making your care more connected, convenient and centered around you.
If you're an active CCH patient, you should have received a sign-up link via the email you provided. If you haven’t received it, are a patient, or want to become one, call the UCHealth MyChart Help Desk at 1-855-467-0810 for assistance.
Option 2: My Health Home
If you would like to access your personal health information for services provided previous to July 18, 2025, you can do so through My Health Home, our previous secure, password-protected patient portal.

Any associated fees will be in accordance with Wyoming state law.
Birth and Death Certificates
Birth and Death certificates are not available from Campbell County Health. These records must be ordered from the Wyoming Department of Health, Vital Statistics Services.
