Medical Records

Campbell County Health is dedicated to providing prompt processing of all medical record requests while ensuring that all medical information is disclosed in accordance with federal and state laws in Gillette, Wyoming.

Here are the options to request your medical records.

Option 1: Online Medical Record-My Health Home

My Health Home allows quick and private 24-hour access to your medical records at no cost from any device with internet access. Learn more about My Health Home

All Behavioral Health & Substance Use Treatment records are not shared via the portal & if the patient needs a copy, to please follow instructions in Option 3.

Option 2: Request Medical Records online (eRequest)

Request medical records via a secure website hosted by our Release of Information partner, Ciox.

At this time, to request any/all Behavioral Health & Substance Use Treatment records, please follow instructions in Option 3.

Monday-Friday, 8 am-4:30 pm. Closed on major holidays.

One courtesy copy will be provided at no charge.

Birth and Death Certificates

Birth and Death certificates are not available from Campbell County Health. These records must be ordered from the Wyoming Department of Health, Vital Statistics Services.

Advance Healthcare Directive

All adults can benefit from thinking about what their healthcare choices would be if they are unable to speak for themselves. You have the right to give instructions about your own healthcare. You also have the right to name someone else to make healthcare decisions for you. Campbell County Health helps you take control of your future healthcare and complete your Advance Directive.
