All Behavioral Health & Substance Use Treatment records are not shared via the portal & if the patient needs a copy, to please follow instructions in Option 3.
Option 2: Request Medical Records online (eRequest)
Campbell County Health has selected a new Release of Information partner that will better serve our community. In the upcoming weeks, you will be able to request medical records through our partner’s portal and receive your records electronically.
In the meantime, please utilize option 3 below for any requests for medical records.

Birth and Death Certificates
Birth and Death certificates are not available from Campbell County Health. These records must be ordered from the Wyoming Department of Health, Vital Statistics Services.

Advance Healthcare Directive
All adults can benefit from thinking about what their healthcare choices would be if they are unable to speak for themselves. You have the right to give instructions about your own healthcare. You also have the right to name someone else to make healthcare decisions for you. Campbell County Health helps you take control of your future healthcare and complete your Advance Directive.
