Registration
Registration is quick and easy, and it's free. You begin your registration process at CCH—be sure to provide a personal email address to use for the account at Registration/Patient Access. From there, patients can self-enroll online (below), or stop at Health and Information Services to create the account.

Enter the following information as it was given at Registration:
Access to the medical record number is available at CCH Health and Information Services—patients must sign paperwork in order to receive their medical record number per HIPAA regulations. The office is open Monday-Friday from 8 am-5 pm. For questions, please call 307-688-7878.

When you sign-up for My Health Home, you can Manage family members' health records, once authorized.
