If you need access to your records from before July 19, 2025, and had not previously registered, click the link below, or stop at Health and Information Services to create the account.

Self-enrollment

Enter the following information as it was given at Registration:

First and Last Name, in all caps
Date of Birth
Personal Email Address
Medical Record Number

Access to the medical record number is available at CCH Health and Information Services.
Patients must sign paperwork in order to receive their medical record number per HIPAA regulations. The office is open Monday-Friday from 8 am-5 pm. For questions, please call 307.688.7878.
